Estimulação transcraniana por corrente contínua e treino motor em hemiparéticos: ensaio clínico, cego e controlado

Summary: | Neurological deficits due to stroke vary in intensity and duration of symptoms, being a major cause of disability and withdrawal from work due to sequelae. Hemiparesis is one of the most characteristic. Aim: To investigate the acute effects of transcranial direct current stimulation (tDCS) associated with a simulation of functional training in hemiparetic patients and healthy volunteers. Methods: A total of 5 hemiparetic subjects (GE) were randomly submitted at different times to Conditions A and B. In both conditions, tDCS was associated with a motor training with paretic limb. In Condition Anodal tDCS was applied on affected cerebral hemisphere and on Condition B, cathodal tDCS was applied on not affected hemisphere. In the group of healthy volunteers (GC), 10 healthy subjects, after randomization, were divided between these two conditions. The following tests were used: Box and Block Test (BBT), Nine Hole and Pin test (9HPT), Handgrip Dynamometry and Electroencephalography (EEG) in three different times: T0, baseline, T1 , after tDCS, and T2, after motor training. Results: In functional measurements, we found statistically difference for BBT between T0 and T1 (p = 0.035) and for 9HPT between T0 and T2 (p=0.038). In GC, a statistically significant difference was found between T0 and T2 in 9HPT (p = 0.044). In the electrophysiology, there was a statistically significant difference in GC during motor imagery in the alpha spectrum between T0, T1 and T2 (p = 0.005) and in the beta band between T0 and T2 (p=0.028) in the non-dominant index movement. Conclusion: The proposed protocol was able to create and identify positive responses of tDCS and motor training in manual dexterity in hemiparetic and healthy subjects. We suggest addition of a sham group, to better evaluate isolated contribution of tDCS and motor training. |

We suggest addition of a sham group, to better evaluate isolated contribution of tDCS and motor training.Os déficits neurológicos decorrentes do AVC podem variar quanto à intensidade e quanto à duração dos sintomas, sendo uma importante causa de incapacidade e afastamento do trabalho devido às sequelas, sendo a hemiparesia uma das mais características. Objetivos: Investigar os efeitos agudos da ETCC associado ao treino motor em voluntários hemiparéticos e voluntários saudáveis por meio de medidas eletrofisiológicas e funcionais. Métodos: Um total de 5 sujeitos hemiparéticos (GE) passou em momentos distintos e conforme aleatorização pelas Condições A e B. Nas duas condições, foi realizada ETCC associada a um treino motor com o membro parético. Na Condição A o indivíduo recebeu estimulação anódica no hemisfério cerebral afetado e na Condição B, este recebeu estimulação catódica no hemisfério cerebral não afetado. No grupo de voluntários saudáveis (GC), 10 indivíduos saudáveis, após randomização, foram divididos entre as duas condições. Para avaliação foram utilizadas as escalas Box and Blocks Test (BBT), teste dos Nove Buracos e Pinos (9HPT), Dinamometria de Preensão Manual (DPM) e Eletroencefalografia (EEG) em 3 tempos distintos: T0, linha de base, T1, pós ETCC, e T2, pós treino motor. Resultados: Nas medidas funcionais, foi encontrado resultado estatisticamente significativo no GE em relação ao membro parético para o BBT entre T0 e T1 (p=0,035) e para o 9HPT entre T0 e T2 (p=0,038). No GC, foi encontrada diferença estatisticamente significativa apenas entre T0 e T2 no 9HPT (p=0,044). Na eletrofisiologia foi encontrada diferença estatisticamente significativa no GC durante a imagética motora no espectro de alfa entre os três tempos (p=0,005) e na banda beta entre T0 e T2 (p=0,028) na movimentação do índex não dominante. Conclusão: O protocolo proposto foi capaz de gerar e identificar respostas favoráveis em relação ao uso da ETCC e do treino motor na destreza manual tanto em voluntários hemiparéticos quanto em sujeitos saudáveis.
