Transferência, Contratransferência e Interpretação da Transferência: regularidades e diferenças entre a psicanálise e a psicoterapia psicanalítica

Este estudo tem como objetivo identificar as caraterísticas dos fenómenos inconscientes transferência e contratransferência, bem como investigar o tipo e contexto da interpretação da transferência e analisar as diferenças da emergência destes fenómenos em duas modalidades díspares de tratamento psicanalítico. Para cumprir este objetivo foram estudadas duas díades (I e II), a primeira em psicoterapia psicanalítica em face-a-face e a segunda em psicanálise cujo processo terapêutico foi seguido em supervisão de grupo. A análise temático-categorial das narrativas em estudo permite identificar diferenças na identificação pré-consciente da transferência e contratransferência e na utilização da interpretação da transferência. Assim, em psicoterapia psicanalítica face-a-face a terapeuta identifica e “acolhe” os fenómenos inconscientes apesar de lhe ser mais difícil “descodificar” o seu significado inconsciente enquanto em psicanálise a representação psíquica destes fenómenos permite que a terapeuta utilize a interpretação da transferência para promover “insight” no paciente sobre a comunicação emocional em sessão. Finalmente tanto a “fluência” da associação livre, como a “atenção flutuante” da terapeuta são favorecidas no tratamento psicanalítico, em particular pela maior frequência das sessões e pela maior intensidade do “diálogo” transferencial-contratransferencial entre terapeuta-paciente. / This work aims to identify the characteristics of the unconscious phenomena transference and countertransference, as well as the type and context of the use of transference interpretation in the therapeutic session across 2 different modalities of psychoanalytic treatment. To achieve such a goal we studied 2 dyads (I and II) the first in psychoanalytic psychotherapy face-to-face and the second in psychoanalysis. The thematic-categorical analyses on the narratives issued from the supervision process showed evidence of differences both in the pre-conscious identification of transference and countertransference and in the use of transference interpretation. Thus, in psychoanalytic psychotherapy face-to-face the therapist feels the transference though he often doesn’t "decrypt" its unconscious meaning while in psychoanalysis the psychic representation of transference allows for the use of transference interpretation to promote insight in the patient about the emotional communication in session. Finally both free association and "floating attention" are enhanced in psychoanalysis mainly due to higher session frequency allowing for a more intense “dialogue” transference-countertransference between therapist and patient.
