Unlike chronic spontaneous urticaria, which occurs without a cause or apparent trigger, chronic inducible urticaria (CIndU) appears after exposure to a specific stimuli. CIndU are divided into physical and nonphysical. Physical CIndU include symptomatic dermographism (triggered by friction), cold urticaria and heat urticaria (triggered by temperature), delayed pressure urticaria (triggered by pressure) and vibratory angioedema (triggered by vibration). Nonphysical CIndU include cholinergic urticaria (triggered by increase in core body temperature), contact urticaria (triggered by contact with chemical compounds) and aquagenic urticaria (triggered by contact with water).Although its pathophysiology is complex and not yet fully understood and described, it is known that the main underlying mechanism is the activation and degranulation of mast cells and basophils with the consequent release of inflammatory mediators. However, little is known about how the CIndU triggers contribute to this phenomenon.The diagnosis of CIndU is based on clinical history and must be confirmed through provocation tests. In addition to their usefulness in confirming diagnosis, provocation tests are used to measure trigger threshold, which is essential for the assessment of disease activity, for advising patients to avoid relevant triggers and for evaluating and monitoring treatment response.Questionnaires that gather all relevant information directly by the patient, the Patient Reported Outcomes, are applied to assess the activity, control and impact of urticaria on the patients' quality of life.Despite its chronic evolution, the majority of patients with CIndU end up achieving spontaneous remission after 1 to 10 years. Therefore, and since there are no curative therapies, the main objective of treatment is symptomatic control. The therapy approach includes identification and elimination of the underlying causes, avoidance of triggers, induction of tolerance and pharmacological treatment. The pharmacological treatment includes non-sedative second generation H1 antihistamines, omalizumab and cyclosporine A.A urticária crónica é uma patologia cutânea frequente que se define clinicamente pelo surgimento de lesões maculopapulares eritematosas e pruriginosas, angioedema ou ambos, com uma duração superior a 6 semanas. Ao contrário da urticária crónica espontânea, que ocorre sem causa ou desencadeante aparente, a urticária crónica indutível (UCInd) surge após exposição a estímulos específicos. As UCInd subdividem-se em físicas e não físicas. Nas UCInd físicas inclui-se o dermografismo sintomático (desencadeado pela fricção), a urticária ao frio e a urticária ao calor (desencadeadas pela temperatura), a urticária retardada à pressão (desencadeada pela pressão) e o angioedema vibratório (desencadeado pela vibração). As UCInd não físicas abrangem a urticária colinérgica (desencadeada pelo aumento da temperatura corporal central), a urticária de contacto (desencadeada pelo contacto direto da pele com determinadas substâncias químicas) e a urticária aquagénica (desencadeada pelo contacto direto da pele com água). Apesar de a sua fisiopatologia ser complexa e ainda não estar totalmente compreendida e descrita, sabe-se que o principal mecanismo subjacente é a ativação e desgranulação de mastócitos e basófilos com consequente libertação de mediadores inflamatórios. Porém, pouco se sabe sobre a forma como os estímulos desencadeantes da UCInd contribuem para esse fenómeno. O diagnóstico de UCInd baseia-se na história clínica e deve ser confirmado através dos testes de provocação. Para além da sua utilidade na confirmação diagnóstica, os testes de provocação são utilizados para determinar o limiar de resposta ao estímulo, que é fundamental para a avaliação da atividade da doença, para o aconselhamento dos doentes à evicção dos estímulos relevantes e para a avaliação e monitorização da resposta ao tratamento.Para a avaliação da atividade, do controlo e do impacto da urticária na qualidade de vida dos doentes são aplicados questionários que reúnem todas as informações relevantes fornecidas diretamente pelo doente, os Patient Reported Outcomes. Apesar da sua evolução crónica, a maioria dos doentes com UCInd acaba por atingir remissão espontânea ao fim de 1 a 10 anos. Assim sendo, e visto que não existem terapêuticas curativas, o principal objetivo do tratamento é o controlo sintomático. A abordagem terapêutica inclui a identificação e eliminação das causas subjacentes, a evicção dos fatores desencadeantes, a indução de tolerância e o tratamento farmacológico.
