Esta entidade engloba um conjunto de alterações histológicas e moleculares complexas dos ductos e lóbulos mamários, cujo diagnóstico definitivo se baseia na classificação histopatológica, imunohistoquímica e molecular. Contudo, as estratégias de diagnóstico atualmente estabelecidas não são capazes de estudar o tumor em toda a sua extensão e, com isso, refletir a sua heterogeneidade.Com o surgimento de terapêuticas cada vez mais dirigidas, a necessidade de instrumentos para diagnóstico, caracterização e monitorização terapêutica destes doentes é progressivamente maior. Por isso, novas combinações de biomarcadores são a base de algoritmos de diagnóstico e seguimento cada vez mais complexos. Neste contexto, a biópsia líquida posiciona-se como uma importante ferramenta na classificação do tipo tumoral, das suas características moleculares e da sua evolução temporal. Esta permite identificar e estudar o tipo e a quantidade de células tumorais circulantes, ácidos nucleicos tumorais, vesículas extracelulares e outros marcadores tumorais em circulação, fornecendo uma visão dinâmica da doença, fora do seu microambiente tumoral, de forma minimamente invasiva e com a possibilidade de obtenção de amostras seriadas. Apesar de permitir acompanhar a resposta terapêutica e detetar o desenvolvimento de resistências de forma rápida e precoce, esta tecnologia ainda não dispensa os métodos de diagnóstico atualmente vigentes. Mesmo não tendo validade clínica completamente reconhecida, nem ser recomendado o seu uso na maioria das guidelines internacionais, a expetativa é de que a biópsia líquida seja usada para deteção de doença em fase precoce ou metastática, monitorização em tempo real e seleção das doentes para os protocolos terapêuticos mais adequados e personalizados.Breast cancer is the most common neoplasm and has the highest mortality rate in women worldwide. This entity includes a set of complex histological and molecular alterations of the breast ducts and lobules, whose definitive diagnosis is based on histopathological, immunohistochemical and molecular classification. However, the diagnostic strategies currently established are not able to study the tumor to its full extent and thereby reflect its heterogeneity.With the emergence of increasingly targeted therapies, the need for tools that allow diagnosis, characterization and therapeutic monitoring of these patients is increasing. Therefore, new combinations of biomarkers are the basis for increasingly complex diagnostic and follow-up algorithms. In this context, liquid biopsy is positioned as an important tool in classifying the tumor type, its molecular characteristics and its temporal evolution. It allows the identification and study of the type and quantity of circulating tumor cells, tumor nucleic acids, extracellular vesicles and other tumor markers, providing a dynamic prespective of the disease, outside its tumor microenvironment, in a minimally invasive way and with the possibility of obtaining serial samples. Although this technology allows monitoring the therapeutic response and detecting the development of resistance quickly and early, it still is not capable to dismiss the diagnostic methods currently used.
