EARLY CHILDHOOD CARIES - PREVALENCE, PATHOGENESIS AND PREVENTIVE APPROACH: Narrative review of the literature

Summary: Early childhood caries (ECC) is defined as a multifactorial and dynamic disease characterized by the presence of one or more carious surfaces (non-cavitated or cavitated lesions), absent (due to caries) or filled, in any deciduous tooth of a child under six years of age. Thus, the objective of this narrative review of the literature was to conduct a survey on the prevalence, pathogenesis and preventive approaches for early childhood caries. Publications by several authors in the format of scientific articles were used, which were available in virtual databases: Scientific Electronic Library Online (SciELO), PubMed and Google Scholar. The keywords used in the search were “early childhood caries”, “caries prevention”, “caries etiology” and pediatric dentistry and the same terms in English “early child caries”, “caries prevention”, “caries etiology” and “pediatric dentistry”. The narrative review of the literature presented brought a reflection on the high prevalence of early childhood caries around the world. Efforts should be made to address the child's condition in the first year of life, determining dietary advice, early hygiene care, rational use of fluoride and treatment techniques based on concepts of minimal intervention dentistry. Given this reality, it is necessary to highlight the relevance of the role of the dentist who treats children in the ongoing guidance of those responsible for the problem in question, contributing significantly to preventing and minimizing the impacts of caries on children's lives.

Por lo tanto, el objetivo de esta revisión narrativa de la literatura fue realizar una encuesta sobre la prevalencia, patogenia y enfoques preventivos de la caries en la primera infancia. Se utilizaron publicaciones de varios autores en formato de artículos científicos, las cuales estuvieron disponibles en bases de datos virtuales: Scientific Electronic Library Online (SciELO), PubMed y Google Scholar. Las palabras clave utilizadas en la búsqueda fueron "caries en la primera infancia", "prevención de caries", "etiología de la caries" y odontología pediátrica y los mismos términos en inglés "caries infantil", "prevención de caries", "etiología de la caries" y "odontología pediátrica". La revisión narrativa de la literatura presentada aportó una reflexión sobre la alta prevalencia de caries en la primera infancia en todo el mundo. Se deben hacer esfuerzos para abordar la condición del niño en el primer año de vida, determinando consejos dietéticos, cuidados de higiene tempranos, uso racional de flúor y técnicas de tratamiento basadas en conceptos de odontología de mínima intervención. Ante esta realidad, es necesario destacar la relevancia del papel del odontólogo que atiende a los niños en la orientación permanente de los responsables del problema en cuestión, contribuyendo significativamente a prevenir y minimizar los impactos de la caries en la vida de los niños.A cárie na primeira infância (CPI) é determinada como uma doença multifatorial e dinâmica caracterizada pela presença de uma ou mais superfícies cariadas (lesões não cavitadas ou cavitadas), ausentes (devido à cárie) ou obturadas, em qualquer dente decíduo de uma criança menor de seis anos. Dessa forma, o objetivo da presente revisão narrativa da literatura foi realizar um levantamento sobre a prevalência, patogênese e abordagens preventivas para cárie na primeira infância. Foram utilizadas publicações de diversos autores na forma de artigos científicos, os quais estavam disponíveis em bases de dados virtuais: Scientific Electronic Library Online (SciELO), PubMed e Google Acadêmico. As palavras-chave utilizadas na busca foram “cárie na primeira infância”, “prevenção de cárie”, “etiologia da cárie” e odontopediatria e os mesmos termos em inglês “early childhood caries”, “caries prevention”, “caries etiology” e “pediatric dentistry”.  A revisão narrativa da literatura apresentada permitiu uma reflexão sobre a alta prevalência da cárie na primeira infância ao redor do mundo. Esforços devem ser realizados para abordar a criança no primeiro ano de vida, determinando aconselhamento dietético, cuidados precoces de higiene, uso racional do flúor e técnicas de tratamento baseado nos conceitos de odontologia de mínima intervenção.
