Preservando a dignidade na plenitude : efeitos da terapia da dignidade em pacientes brasileiros em cuidados paliativos

Samenvatting: | Despite the growing implementation of palliative care (PC) in Brazil, there is a significant lack of evidence regarding specific psychotherapeutic interventions for existential suffering in terminally ill patients. This dissertation aimed to evaluate the effectiveness of Dignity Therapy (DT) in patients receiving PC, using a quasi-experimental design, without a control group, and a mixed-methods approach, integrating both quantitative and qualitative data.The study included four participants, aged between 38 and 77 years, diagnosed with life-threatening illnesses, from two institutions in Santa Catarina, between August 2024 and April 2025. The intervention followed the semi-structured DT protocol, which seeks to promote the re-signification of life trajectories and the preservation of dignity.To measure the effects, the Patient Dignity Inventory (PDI-Br) and the Hospital Anxiety and Depression Scale (HADS) were applied before and after the intervention. Simultaneously, interviews were analyzed using Descending Hierarchical Classification and Similarity Tree (IRaMuTeQ).All participants showed a reduction in PDI-Br scores (ranging from 12 to 21 points), while anxiety and depression indicators remained stable at mild or absent levels. The qualitative analysis identified five thematic categories: family bonds and emotional support (27.1%), illness process (21.7%), spirituality (19.9%), re-signification and hope (18.1%), and affective memories (13.1%). Despite limitations related to the small sample size and specific cultural context, the findings reinforce the therapeutic potential of DT in the context of Brazilian palliative care, highlighting its contribution to comprehensive care that preserves dignity and personal identity at the end of life. |
